South Westminster Recovery Service

Community Recovery Teams bring together a range of health and social care professionals who each have specialist skills.

They are usually teams of around 40 people. Each team has several types of workers with different knowledge and skills. They will each understand how the others work and will understand how to tackle problems together.

Professions within the community recovery team will include:

  • Psychiatrists
  • Approved Mental Health Practitioners (AMHPs)
  • Community psychiatric nurses (CPN)
  • Social workers
  • Occupational therapists (OT)
  • Clinical psychologists
  • Pharmacists

Other members of the team include:

  • Team manager
  • Clinical Team Leaders
  • Receptionists and Secretaries
  • Other staff such as support workers, mental health workers, employment specialists, art therapists and psychotherapists

All the team members will understand the distress that goes with mental illness. They can all offer psychological support, encouragement and practical help.

The team will provide integrated social, physical and mental health care and work closely with local primary care teams and community providers. The majority of service users will be under the Care Programme Approach and will have a Care Co-ordinator and a care plan. The care plan is based on the recovery goals the service user wants to achieve, which is regularly reviewed. The teams also provide carers assessments and support. They will continue to work with service users under Lead Professional Care where the service user is likely to see just one member of the multi-disciplinary team.

Referrals can be made by GPs and other mental health professionals.
