A crisis card will indicate to someone that you are a user of mental health services. It is useful in a crisis when you are too ill to give useful information which would help other people to help you. It is a small card that you can carry around with you in your wallet.
The card has space to write the telephone numbers of people who you would want contacted in an emergency. It also has space to enter the telephone numbers of your keyworker, GP and hospital. It has the following headings where you can enter other important information: allergies; other medical conditions; dietary needs; special requests. There is space for you to enter your medication if you wish to do so.
You can get a crisis card from your keyworker or from a Recovery Team.
